A Required Health Screening, Done in Minutes Instead of Days
MyHealthForm.com is an online platform that gives Florida home health caregivers the state-required “free from communicable disease” health statement without a doctor’s visit. Created by DSNMRK partner Shae Strachan and his brother Scott Strachan, RN, it turns a slow, expensive step in caregiver onboarding into a nurse-reviewed, physician-approved certificate a caregiver can complete while sitting in the registry’s office. DSNMRK designed, built and runs the platform.
— The Story
Filling a void in Florida home care onboarding
In Florida, a home health worker must have a health statement confirming they are free from communicable diseases before they can enter a client’s home. For most caregivers that meant booking a visit with a primary care physician, waiting for the appointment, paying for it and then waiting again for the paperwork. Many physicians were reluctant to sign the statement at all. Nurse registries watched good caregivers sit idle for days or weeks while a single form held up their start date.
Scott Strachan, RN, BSN, sees that problem every week at Abby Services, the nurse registry our father founded in 1997. Shae Strachan, a partner at DSNMRK, had already built C-E-U.com with him. Together they saw a second opportunity to help the caregiving community: a compliant, physician-approved screening a registry could run on the spot. That idea became MyHealthForm.com.

— The Challenge
Fast for caregivers, defensible for registries
- 1The screening had to satisfy Florida Statute 400.506(6)(a) and hold up under an Agency for Health Care Administration (AHCA) survey. That meant a physician-approved protocol, issued and reviewed by a registered nurse, with nothing improvised.
- 2It had to be fast enough to complete during onboarding: register, pay, answer the screening questions and receive a signed certificate while the caregiver is still in the office.
- 3Certificates had to be tamper-proof and verifiable, so a registry, and a surveyor, can confirm any form is genuine in seconds and falsified screenings are no longer a risk.

Built mobile-first
— What We Built
A compliant screening platform, start to certificate
01
Register and pay in minutes
A caregiver creates an account, enters basic information and pays securely from any phone or the registry’s front desk. No appointment, no exam, no delay.
02
A physician-approved screening protocol
The screening follows a written protocol approved by a physician and issued by a registered nurse, so the process itself, not just the form, meets AHCA expectations for Florida nurse registries.
03
Instant signed certificate
On completion the platform generates the physician-approved, signed “free from communicable disease” certificate immediately, ready to file in the caregiver’s onboarding packet.
04
Online certificate verification
Every certificate can be validated on the site, giving registries and surveyors a fast way to confirm authenticity and protecting patients from falsified screenings.
05
Built for nurse registries
Designed around how Florida nurse registries onboard independent caregivers, with referral partnerships and a neutral third-party process that keeps registries clear of contractor-control risk.
06
Educational content that ranks
Plain-language articles on what the statute requires, what surveyors look for and how to avoid common compliance mistakes, structured to be found by search engines and AI assistants.
— The Outcome
Days of waiting, replaced by a few minutes
Minutes
To go from registration to a signed certificate, while the caregiver is still in the office
$0 visits
No physician appointment, no exam and no waiting on a clinic for paperwork
Survey-tested
Health statements reviewed during AHCA surveys at Florida nurse registries with no change required
Faster onboarding
Caregivers start working sooner and registries fill shifts faster, because one form no longer holds everything up.
Compliance by design
A nurse-reviewed, physician-approved protocol built around Florida’s rules, not bolted on afterwards.
Verifiable, every time
Online verification means registries, agencies and surveyors can trust every certificate they see.
Good for the community
Less friction for caregivers and safer, better-documented care for the families who rely on them.
We built it because we needed it ourselves. Now every Florida registry can use it.

A Training Platform Built by a Nurse and a Technologist, Now 30,000 Users Strong
C-E-U.com is an online continuing education and certification platform for nurses, nurse aides and caregivers: state board-approved courses, instant certificates and simple, affordable pricing. It was created by DSNMRK partner Shae Strachan and his brother Scott Strachan, a registered nurse who runs the home health agency their father founded. DSNMRK designed, built and grew the platform, which has passed 30,000 users in a short period of time.
— The Story
Two brothers, one idea that helps the people who care for others
C-E-U.com is near and dear to us. Scott Strachan, RN, BSN, is a registered nurse and healthcare entrepreneur who owns and operates Abby Services, the Florida nurse registry our father started in 1997. Every year he watched caregivers and nurses scramble to complete the continuing education their state requires, paying too much for courses that were slow, dated and hard to use.
Shae Strachan, a partner at DSNMRK, has spent two decades building digital platforms and business automation. When the brothers compared notes, the opportunity was obvious: combine Scott’s knowledge of caregiver compliance and home care operations with Shae’s technology and marketing experience, and build the training platform the healthcare community actually needed. C-E-U.com was born.

— The Challenge
Make required training fast, affordable and trustworthy
- 1Continuing education requirements differ by state and by role. Caregivers, CNAs, HHAs, RNs and LPNs each needed a clear path to exactly the courses their board requires, without wading through everything else.
- 2Certificates have to be instant, verifiable and reportable. Employers and state boards need to trust every certificate the platform issues, and where a state uses CE Broker, completions need to be reported automatically.
- 3The platform had to launch quickly, then scale. Home care agencies enroll staff in bulk, and a nurse finishing a night shift expects checkout, course and certificate to just work on a phone.

04

A Resort Website That Fills Tee Sheets, Ballrooms and Wedding Dates
Pala Mesa Resort is a Southern California golf resort in Fallbrook with a championship course, hotel accommodations, AquaTerra Restaurant, meeting space and one of the region’s most sought-after wedding venues. DSNMRK designed, built, hosts and maintains palamesa.com: an easy-to-update, highly AI and search optimized site that has helped drive more golf bookings, private events and weddings.
— The Client
Four businesses on one property
Pala Mesa is really four businesses sharing one beautiful piece of land: a public championship golf course with daily tee times, tournaments and lessons; a hotel with rooms and room blocks; AquaTerra, a restaurant and bar with live music and happy hours; and a wedding and events operation that hosts ceremonies, receptions, corporate meetings and celebrations year round.
Each one has its own audience, its own seasonality and its own way of booking. The website has to sell all of them at once, and it has to be simple enough that the resort’s own team can keep it current every week.

— The Challenge
Be found for golf, weddings and events, then convert
- 1Golfers, couples and event planners all search differently. The site had to rank, and be recommended by AI assistants, for tee times, wedding venues and private events across North San Diego County.
- 2Every visitor needed a clear next step: book a tee time, request a wedding tour, submit an event proposal, reserve a room or a table, without hunting through the site.
- 3Specials, packages, menus, live music and seasonal events change constantly, so the marketing team needed to publish updates themselves, without a developer.

A Resort Website Built to Perform, and to Be Found by Search and AI
Chamonix Casino Resort is Colorado’s premier casino resort in Cripple Creek, an hour from Colorado Springs, with luxury rooms and suites, slots and table games, 980 Prime Steakhouse, a rooftop pool and a full spa. DSNMRK designed and built chamonixco.com as a dynamic, high-performance site that is heavily optimized for search engines and AI assistants, with custom hotel booking widgets, custom AI-generated video animations, and a robust CMS the resort’s non-technical staff update themselves, all while meeting accessibility and privacy standards.
— The Client
A new luxury destination with a lot to say
Chamonix is the newest luxury casino resort in the Colorado mountains: a hotel, a casino floor, fine dining, a spa and salon, live entertainment and meeting space, all under one roof in historic Cripple Creek. A property like that is never finished talking. Promotions, progressives and player rewards change weekly, entertainment is booked months out, and menus, packages and seasonal offers turn over constantly.
The website had to launch as the resort’s front door, carry every one of those messages, convert visitors into hotel guests, and be discovered by the people searching for a Colorado getaway, whether they ask Google or an AI assistant.

— The Challenge
Launch fast, book direct, and keep it easy to run
- 1A brand-new destination has no search history. The site had to earn visibility quickly in traditional search results and in the AI-generated answers travelers increasingly rely on.
- 2Booking a room should never mean leaving the experience for a clunky third-party portal. Guests needed a way to start a reservation from any page in a few taps.
- 3A lean marketing team with constant change needed to publish promotions, events and menus themselves, without a developer and without breaking accessibility or privacy compliance.

One Platform for the Whole Resort, Built to Run Itself
Pala Casino Spa Golf Resort is a destination casino resort in Southern California with a full casino floor, a hotel and spa, championship golf, a dozen restaurants and bars, and a packed entertainment calendar. DSNMRK designed, built and hosts palacasino.com, and built the back-end tools and automations that let Pala’s marketing team run all of it without waiting on a developer.
— The Client
A resort that changes every single week
Pala is one of Southern California’s largest gaming and entertainment destinations. A property that size produces a constant stream of change: new promotions and giveaways, headline concerts and Center Bar bands, seasonal restaurant menus, hotel and golf offers, and player rewards that shift by the day.
Every one of those changes has to reach guests in three places at once, on the casino floor, on the website and in their inbox, quickly and without mistakes. DSNMRK has managed, hosted, maintained and extended Pala’s digital platform to make that the easy part of the marketing team’s week.

— The Challenge
Legacy systems that cost more than they delivered
- 1The website and its supporting tools ran on a patchwork of legacy platforms, each with its own licensing fee, its own vendor and its own way of doing things.
- 2Routine updates such as a band change, a new menu or a fresh promotion waited on vendors or developers, so marketing moved slower than the property did.
- 3Gaming entries, giveaways and email campaigns were handled by hand, which meant duplicated effort, delays and room for error.

A custom admin experience lets the casino marketing team publish events, schedule concerts and bands, update restaurant menus and manage promotions in minutes. No tickets, no developer, no vendor.
03
Automated gaming entries and giveaways
Promotional entries, drawings and giveaways run on rules the team sets once. Entries are collected, validated and tracked automatically, so the team spends its time on the promotion, not the paperwork.
