MyHealthForm.com is an online platform that gives Florida home health caregivers the state-required “free from communicable disease” health statement without a doctor’s visit. Created by DSNMRK partner Shae Strachan and his brother Scott Strachan, RN, it turns a slow, expensive step in caregiver onboarding into a nurse-reviewed, physician-approved certificate a caregiver can complete while sitting in the registry’s office. DSNMRK designed, built and runs the platform.

Filling a void in Florida home care onboarding
In Florida, a home health worker must have a health statement confirming they are free from communicable diseases before they can enter a client’s home. For most caregivers that meant booking a visit with a primary care physician, waiting for the appointment, paying for it and then waiting again for the paperwork. Many physicians were reluctant to sign the statement at all. Nurse registries watched good caregivers sit idle for days or weeks while a single form held up their start date.
Scott Strachan, RN, BSN, sees that problem every week at Abby Services, the nurse registry our father founded in 1997. Shae Strachan, a partner at DSNMRK, had already built C-E-U.com with him. Together they saw a second opportunity to help the caregiving community: a compliant, physician-approved screening a registry could run on the spot. That idea became MyHealthForm.com.
— The Challenge
Fast for caregivers, defensible for registries
- 1The screening had to satisfy Florida Statute 400.506(6)(a) and hold up under an Agency for Health Care Administration (AHCA) survey. That meant a physician-approved protocol, issued and reviewed by a registered nurse, with nothing improvised.
- 2It had to be fast enough to complete during onboarding: register, pay, answer the screening questions and receive a signed certificate while the caregiver is still in the office.
- 3Certificates had to be tamper-proof and verifiable, so a registry, and a surveyor, can confirm any form is genuine in seconds and falsified screenings are no longer a risk.

It was created by DSNMRK partner Shae Strachan and his brother Scott Strachan, a registered nurse who runs the home health agency their father founded. DSNMRK designed, built and grew the platform, which has passed 30,000 users in a short period of time.

Two brothers, one idea that helps the people who care for others
C-E-U.com is near and dear to us. Scott Strachan, RN, BSN, is a registered nurse and healthcare entrepreneur who owns and operates Abby Services, the Florida nurse registry our father started in 1997. Every year he watched caregivers and nurses scramble to complete the continuing education their state requires, paying too much for courses that were slow, dated and hard to use.
Shae Strachan, a partner at DSNMRK, has spent two decades building digital platforms and business automation. When the brothers compared notes, the opportunity was obvious: combine Scott’s knowledge of caregiver compliance and home care operations with Shae’s technology and marketing experience, and build the training platform the healthcare community actually needed. C-E-U.com was born.
